Following survey completion, those choosing to enter a drawing to win one of four $150 gift cards may do so by completing the required entry form sections below, indicated by a red asterisk (*). Enter your CONTACT INFORMATION and the CODE which was provided at the end of the survey. Your survey responses will remain anonymous even with a survey entry. Four (4) winners of one gift card valued at $150 each will be randomly selected following the close of the survey.

    If you do not receive Andrea's outreach announcements and would like to be added to her contact list, please check the method of delivery that you prefer. Email is recommended for timeliness and there may be an annual mailer subscription fee due to postage, supplies, and other costs of preparing and sending mailers. Provide a mailing address below if you prefer a mailer.
    Optional: ​Help us receive additional coffee needs assessment responses. Include a fellow coffee farmer, manager, worker, or producer's name and email below, and receive an (1) additional drawing entry.
